Description
Gateway Foundation Corrections is dedicated to rebuilding the lives of incarcerated individuals struggling with substance use disorders. Re-Entry Caseworkers are responsible for:

• Working with clients with co-occurring mental health and substance abuse disorders to coordinate the transition from incarceration into the community
• Conducting assessments of clients’ social, financial, medical and environmental status and needs
• Coordinating participant transition to community, including acting as liaison with program staff, parole/probation, and service providers
• Acting as a resource for variety of community services
• Completing discharge follow-up contacts
• Maintaining necessary client records and documentation, including preparing related summary reports, as required
• Other duties as assigned



Requirements
Requirements:
• Experience providing case management and community support services to persons with substance abuse and/or co-occurring disorders
• Bachelor’s degree in a human services area (social work, human services, criminal justice, psychology) or an equal number of years of experience providing case management services
• Experience working with corrections involved clients/offenders is preferred
• Must have a valid driver's license
• Must be able to obtain Missouri Department of Corrections clearance
• This is a full-time position working Monday - Friday, daytime hours. The selected candidate will work in the Booneville Correctional Center 1 day per week and the remaining 4 days in the Kansas City office.
